Gene regulation, specifically regulation of the leptin receptor and JAK/STAT signal transduction pathway can also influence obesity. Leptin is the product of the obese (ob) gene and is produced predominantly in white adipose tissue. The leptin receptor (Ob-R) isolated by expression cloning, is encoded by the diabetes (db) gene. The arcuate nucleus (ARC) of the hypothalamus serves as the leptin signaling center. Leptin receptor activation stimulates STAT1, STAT3 and STAT5 tyrosine phosphorylation in vitro and STAT3 tyrosine phosphorylation in vivo. The Ob-Rb, long leptin receptor isoform, acts as a potent JAK/STAT signaling activator. Ciliary neurotrophic factor (CNTF) appears to act similar to leptin. CNTF administration has been shown to reduce adiposity much like leptin. Systemic administration of CNTF activates the JAK/STAT pathway in the ARC. Unfortunately, Axokine an analog of CNTF, showed disappointing results in clinical trials for the treatment of obesity. However, there is still promise for gene regulation therapies in the treatment of obesity.
