# 热原检测方法
致热原检测用于测定注射用药品或生物制剂中的致热原含量，并受美国食品药品监督管理局（FDA）、《美国药典》（USP）或《欧洲药典》（EP）等机构制定的多项标准监管。 产品的无菌性并不意味着其不含热原。因此，即使是标称无菌的药物，也必须进行热原检测，以防止患者出现发热反应。
热原污染可能发生在药品、生物治疗制剂和医疗器械的生产或给药过程中，但热原的存在也可能是产品的固有特性，例如疫苗中的佐剂或合成脂肽。

致热原是指通过激活先天免疫系统，导致人类或动物体温升高（发热反应）的物质。它们是一类由微生物和非微生物物质组成的异质性污染物。致热原可分为两类：内毒素和非内毒素致热原（NEPs）。
内毒素是革兰氏阴性菌中存在的一种物质。非内毒素致热原则是其他微生物物质，包括源自革兰氏阳性菌、病毒、酵母菌和真菌的物质。非微生物致热原物质也可能源自橡胶颗粒、微小的塑料颗粒或弹性体中的金属化合物。
目前有多种检测致热原的方法。根据其检测的污染物类型以及是否需要使用动物材料进行测试，这些方法可分为以下几类，具体如下表所示：

兔致热原试验（RPT）是通过测量兔子在静脉注射受测产品后体温的升高情况来进行检测的。该试验仅提供定性结果，且灵敏度较低。此外，由于兔子在反复注射后会产生致热原耐受性，或者在进行试验时受到应激影响，该试验的可靠性也受到一定限制。
注：欧洲现已禁止使用兔热原试验（RPT），单核细胞活化试验（MAT）已成为检测内毒素和非内毒素热原的首选方法。

[单核细胞活化试验（MAT）](https://www.sigmaaldrich.com/SG/zh/products/industrial-microbiology/pyrogen-testing)是检测内毒素和非内毒素热原的一种替代动物实验的方法。该试验通过将单核细胞与待测样本共同孵育，模拟人体免疫反应。若存在热原，单核细胞将被激活并产生导致发热反应的炎症分子——细胞因子。 随后，通过使用特异性抗体和酶促显色反应的免疫学检测法（ELISA）来检测这些细胞因子。
注：自2025年7月起，欧洲药典（Ph. Eur.）委员会建议将单核细胞活化试验（MAT）作为内毒素和非内毒素致热原检测的首选方法。兔致热原试验（RPT）现已在欧洲被禁止。

内毒素检测最常见的方法是鲎血细胞裂解物试验（LAL试验）；该试验基于鲎血细胞裂解物。鲎血细胞裂解物会与细菌内毒素发生凝血反应。该方法对内毒素的定量检测具有高灵敏度，但无法检测非内毒素致热原。

重组因子C是一种基因工程蛋白，通常存在于鲎血细胞裂解物凝血级联反应中。在此检测中，因子C与内毒素发生反应，并与标记物结合，产生可定量的荧光终产物。重组因子C检测采用与LAL检测相同的原理，但无需使用动物源性材料。
重组级联试剂（r-CR）是一种基因工程制备的多组分试剂，可再现鲎血细胞裂解液中完整的凝血级联反应。在此试验中，级联蛋白（C因子、B因子和前凝血酶）依次与内毒素反应，生成可定量的终产物。 r-CR 检测再现了完整的 LAL 途径，但无需使用动物源性材料。
