# 熱原檢測方法
致熱原檢測旨在測定非口服藥品或生物製劑中的致熱原含量，並受美國食品藥物管理局（FDA）、《美國藥典》（USP）或《歐洲藥典》（EP）等組織所制定的多項標準規範。 產品的無菌性並不意味著其不含熱原。因此，標榜為無菌的藥物也必須進行熱原檢測，以防止患者發生發熱反應。
熱原污染可能發生於藥品、生物療法製劑及醫療器材的生產或使用過程中，但熱原的存在也可能是產品的固有特性，例如疫苗中的佐劑或合成脂肽。

致熱原是指能透過激活先天免疫系統，導致人類或動物體溫升高（發熱反應）的物質。它們構成了一類異質性的污染物群，包含微生物及非微生物物質。致熱原可分為兩大類：內毒素與非內毒素致熱原（NEPs）。
內毒素是存在於革蘭氏陰性菌中的物質。非內毒素性致熱原則是其他微生物物質，包括源自革蘭氏陽性菌、病毒、酵母菌及真菌的物質。非微生物性致熱原物質亦可能源自橡膠微粒、微觀塑膠微粒，或彈性體中的金屬化合物。
目前有多種檢測致熱原的方法。這些方法可根據其檢測的污染物類型，以及進行測試時是否需要使用動物材料來分類，詳見下表：

兔內毒素試驗（RPT）是透過測量兔子在靜脈注射受測產品後體溫的上升幅度來進行。RPT 提供的是定性結果，且其敏感度相當低。此外，由於兔子在反覆注射後會產生內毒素耐受性，或是在進行試驗時受到壓力，導致此試驗的可靠性亦受到限制。
註：歐洲現已禁止進行兔子致熱原試驗（RPT），而單核細胞活化試驗（MAT）現為檢測內毒素及非內毒素致熱原的首選方法。

[單核細胞活化試驗（MAT）](https://www.sigmaaldrich.com/TW/zh/products/industrial-microbiology/pyrogen-testing)是檢測內毒素及非內毒素致熱原時，可替代動物實驗方法的一種替代方案。單核細胞活化試驗透過將單核細胞與待測樣本共同培養，模擬人體的免疫反應。若樣本中存在致熱原，單核細胞便會被活化並產生引發發熱反應的發炎分子——細胞因子。 隨後，透過使用特異性抗體及酶色反應的免疫學檢測（ELISA）來檢測這些細胞因子。
註：自 2025 年 7 月起，歐洲藥典（Ph. Eur.）委員會建議將單核細胞活化試驗（MAT）作為內毒素及非內毒素致熱原檢測的首選方法。兔致熱原試驗（RPT）現已在歐洲被禁止使用。

內毒素檢測最常見的方法是鱟血細胞裂解物試驗（LAL 試驗）；這是一種基於鱟血細胞裂解物的檢測法。鱟血細胞的裂解物會與細菌內毒素自然發生凝血反應。此方法對內毒素的定量具有高靈敏度，但無法檢測非內毒素致熱原。

重組因子 C 是一種基因工程蛋白質，通常存在於鱟血細胞裂解液的凝血級聯反應中。在此檢測中，因子 C 與內毒素發生反應，並與標記物結合，產生可量化的螢光終產物。重組因子 C 檢測採用與 LAL 檢測相同的原理，但無需使用動物來源的材料。
重組級聯試劑（r-CR）是一種經基因工程改造的多組分試劑，可重現鱟血細胞裂解液中完整的凝血級聯反應。在此測試中，級聯蛋白質（因子 C、因子 B 及前凝血酶）會依序與內毒素反應，產生可量化的終產物。 r-CR 檢測重現了完整的 LAL 途徑，但無需使用動物來源的材料。
