Y-27632 is a reversible, cell-permeable, ATP-competitive inhibitor of ROCK-I and ROCK-II (Ki~0.2–0.3 μM) that targets the catalytic cleft to fundamentally modulate cytoskeletal mechanics. By blocking actomyosin contractility, it induces the disassembly of actin stress fibers and focal adhesions, preventing membrane blebbing and driving potent smooth muscle relaxation (IC50~0.3–1.0 μM). While effective in hypertensive models, its systemic utility is constrained by rapid clearance and the risk of profound hypotension, often necessitating local administration. Consequently, its therapeutic potential is best realized in specialized tissues: Y27632 crosses the blood-brain barrier to promote axon regeneration and prevent growth cone collapse in CNS injury, and in ophthalmology, it lowers intraocular pressure and aids corneal endothelial healing. Beyond these in vivo applications, Y-27632 is the industry standard for ex vivo cell manipulation, particularly in cancer biology to inhibit metastasis and, most significantly, in stem cell research. Here, Y-27632 prevents dissociation-induced apoptosis (anoikis) in human pluripotent stem cells (hESCs/iPSCs), revolutionizing single-cell cloning, enzymatic passaging, and graft viability in regenerative medicine.
